Is good care at home for seniors with chronic health conditions possible? At what cost? To whom? February 2011

The paper draws on recent data on home-care use in Ontario mined by Prof. John Hirdes and his University of Waterloo research team. It examines Ontario seniors with chronic health conditions to help us understand how they use home-care services, what their health-care needs are as they move from hospital to home and/or community care, and what the implications are for their informal caregivers.

The Essential Guide for Timely and Effective Use of Therapy Services in Homecare January 2010

Developed by the Alliance of Professional Associations for Community-Based Therapy Services(APACTS), this document is a tool to assist in decision-making for case-managers, when referring for therapy services (Dietetics, Occupational Therapy, Physiotherapy, Social Work, Speech-Language Pathology). Included is a reference entitled: "Linking the therapies to the MDS-HC", which identifies the therapy that can address specific Client Assessment Protocols (CAPs).
